Definitely the dragon transformations of Breath of Fire. Does body switching count at all?
There's a computer game I played where you wake up as a cyborg with no memory and through the course of the game find out who you were(who you were depended on things you did during the game) called Bioforge, which I enjoyed immensely.
I bet, if it had made the transition to US shores, that I would have enjoyed Project Altered Beast a lot.
My favorite transformation game, though, would have to be...Bad Mojo: The Roach Game. Yes! Get your "Ewwww"s ready, it's a game where you play an entomologist named Roger Samms who is living in an apartment above a seedy waterfront bar and he's about to make for the islands with his research grant. He's just about to go when he realizes he almost forgot a locket in the form of an insect totem that was supposed to be his mother's(she died during child birth and his father gave him up for adoption).
When he picks it up it starts to glow and there's a flash of light and a crackle of energy and a cockroach aura surrounds him. He screams and falls to the ground, unconscious. When he awakens he's a cockroach hiding in a drain in one of the bar/apartment's rooms. From this point on you play him as a roach on a journey to piece together his past and, hopefully, discover what is truly important in life.
The game is full of friends and dangers. Friends in the form other insects and various animals who will help you through the quest as well as deadly spiders, a roaming cat and a vicious rat, among others. And, of course, your landlord. Hey, you can crawl all over his face while he's asleep! As a matter of fact...you can crawl EVERYWHERE in this game.
